Office INformation and Policies

We welcome you to our new, state of the art facility. We would like to make your visit a pleasurable one. Here’s some information and a few tips to ensure that.

Office hours are 8:15 a.m to 5 p.m. Monday through Friday with some Saturday hours available. The office is closed for lunch from 12:00 p.m. to 1:15 p.m.

All prescription refill requests are processed at the end of business day. Please have your pharmacy fax your requests to (310) 534-9112. Requests will be processed within 24 to 48 hours.

New patients are encouraged to complete the registration process on-line, prior to your appointment, using our patient portal . Those who choose not to register on-line, will need to arrive 30 minutes prior to your appointment time to complete the necessary registration forms.

Please bring your current insurance card and an appropriate photo I.D. ( driver’s license or state I.D.).

Patients that need a referral should have a current authorization on file prior to your appointment.

Returning patients should notify us of any changes in name, insurance and address prior to your visit.

Co-payments and deductibles are collected at the time of registration prior to your visit.

Minors without parents will require written parental consent at the time of their visit.

Cancellation Policy

Patients that do not cancel their appointment within 24 hours of their office visit are subject to a $25.00 cancellation fee.
